Use this form at enrolment time if you think you might be eligible for Credit. Please read instructions for completing form and ensure you discuss your application with your Teaching Department. Deadlines for submission apply.

Application for Re-Crediting HELP & VET Student Loans in Special Circumstances

This form is only applicable for students enrolled in a VET Diploma, VET Advanced Diploma, VET Graduate Certificate, VET Graduate Diploma or a Higher Education program.

Application for Re-mark or Re-appraisal Form

Use this form to request a re-mark of an examination.

Application for Special Consideration or Deferred Assessment

For Special Consideration, the form must be completed and submitted along with all supporting documentation within five (5) working days after the completed assessment.
For Deferred Assessment, the form must be completed and submitted along with all supporting documentation within five (5) working days prior to assessment due date.

Disclosure of Information Permission Form

Use this form if you consent to the disclosure of relevant information between Holmesglen and an associated third party.

Personal Details Amendment Form

Use this form for change of personal details. For change of name attach certified copy of Change of Name Registration or Marriage Certificate.
Address and phone details may also be updated online. Login to Student account at my.holmesglen.edu.au and select "Personal Details" to update address and contact details.
